ODR Data Entry

PBIS-MN evaluates the degree to which implementing PBIS affects changes in the social climate of schools and decreases problem behaviors in schools.

PBIS-MN requests that schools enter basic information about their school, as well as Office Discipline Referral (ODR) and suspension and expulsion data for the 2015-2016 school year; starting in August/September 2015 (beginning of the school year) through June 2016 (end of the school year).

This data entry system is intended for schools that enter ODRs into a system other than SWIS (such as TIES, Infinite Campus, Skyward, CLASSROOM, etc.). Your school does NOT need to enter data into this data entry system if your school already uses SWIS.

Before you get started

It's important for you to know how PBIS-MN defines Major and Minor ODRs.

MAJORS: MAJORS are behavior incidents that are office-managed (such as with a behavioral specialist, principal, or other staff).

MINORS: MINORS are behavior incidents that are classroom-managed (such as with a teacher, educational assistant, or other staff).

Here's what you will need to complete this form:

  1. Total student enrollment (the official number reported to MDE on October 1)
  2. Number of school days from the beginning to the end of the school year.
  3. The system your school is using to track and enter ODR data.

Once you have gathered the above data, please answer the questions on this entry form to generate triangle data. A summary graph will be produced after all scores have been entered and the data has been submitted.
